      - Note: NHLBI changed their procedure between the time we created our original memo and the time the data became available. They are no longer requiring signatures from each user of the data. Instead, the requestor and the signing official of the institution electronically sign the application online and by signing they are affirming that each of the individuals listed on the application has read and agreed to the data use certificate and has had human subjects training.
